$50 Same-Day Medical Telehealth Visit | Non-Psychiatric
$50 self-pay virtual visit for eligible Florida patients with nonemergency medical concerns, minor illnesses, or appropriate routine medication-refill requests. This service does not include psychiatric evaluations, mental-health medication management, FMLA documentation, weight-management treatment, controlled substances, or narcotics. Prescriptions are provided only when medically appropriate. Call 911 or seek emergency care for urgent or life-threatening symptoms.
Self-Pay Psychiatric Telehealth Visit | Florida
$150 per virtual visit. Meet with Dr. DaVida Miller DNP, for an individualized psychiatric evaluation or medication-management appointment. Your symptoms, history, current medications, life stressors, and treatment goals will be reviewed to develop a personalized plan. Treatment may include medication management, education, coping strategies, and follow-up recommendations. Prescriptions are provided only when clinically appropriate.
Mental Health Evaluation for FMLA | $249
$249 per evaluation. This service includes a comprehensive psychiatric assessment, review of your symptoms and functional limitations, treatment planning, and consideration of FMLA documentation. Purchasing an evaluation does not guarantee that FMLA certification will be clinically supported or approved. Records, assessments, established treatment, and additional visits may be required. Final approval is determined by your employer or benefit administrator.
